  • Effective collaboration between Child Safety and Early Action Group members to provide relative Child Safety information regarding young people and their family engaged with EAG.

  • Conduct initial assessments and develop family focused case plans, including referrals to mitigate risk factors and address criminogenic needs.
  • Successful transition of EAG participants including a reduction in offending behaviour.

  • Key Responsibilities
  • Conduct an initial assessment of a young person using the Youth Level of Service/Case Management Inventory: Screening Research Version (YLS/CMI: SRV) to assess a young person’s risk and needs to reduce offending.
  • Assess the family/household to understand the family’s needs impacting on family functioning, young person’s behaviour.
  • Develop individual case plans for the young person/family which identifies specific goals and actions to address the identified criminogenic needs, in addition to providing support to the family.
  • Identify and refer the young person/family to appropriate community resources and support services based on identified needs.
  • Continuously review the young person/families case plan progress and review the case plans as needed.
  • Maintain accurate records of client interactions and progress.
  • Work across the Early Action Group to provide a coordinated service response.
  • Consult and effectively work with relevant stakeholders including families, Elders, other key community members, community agencies and government departments to enhance their capacity to support young people and their families.
  • Regular engagement and interface with community service sector
  • Provide specific Child Safety advice and information that is required to support service delivery for a young person and their family engaged with the Early Action Group in accordance with legislative requirements.
