- Key Responsibilities
- Deliver compassionate personal care including showering, dressing, grooming, toileting, continence care and personal hygiene while maintaining each client's dignity, privacy and independence.
- Support clients with dementia through reassurance, meaningful engagement and person-centred behaviour support strategies.
- Provide compassionate palliative and end-of-life care, ensuring comfort, dignity and emotional support for clients and their families.
- Assist with medication administration in accordance with organisational policies, care plans and scope of practice.
- Prepare nutritious meals and provide feeding assistance, including PEG feeding where required.
- Monitor clients' nutritional intake and hydration while reporting concerns promptly.
- Safely assist with mobility, repositioning and transfers using hoists, slide sheets, standing lifters, wheelchairs and safe manual handling techniques.
- Support clients to attend medical appointments, shopping, community outings and social activities to promote independence and community participation.
- Observe, document and promptly report changes in clients' physical, emotional and cognitive wellbeing.
- Complete accurate progress notes, incident reports and electronic care documentation.
- Build positive relationships with clients, families and carers through compassionate, respectful communication.
- Collaborate effectively with Registered Nurses, Enrolled Nurses, Care Managers, Allied Health professionals and multidisciplinary teams to ensure continuity of care.
- Maintain strict confidentiality while complying with infection prevention procedures, Aged Care Quality Standards and Workplace Health & Safety requirements.
