I am a dedicated and committed RN works in MAPU(medical assessment and planning unit)since 2016. I have also undertaken Acting CN role in MAPU for the past few years. i have foster effective communication and work in partnership with patients and all health care providers having proven ability, skill and experience in advanced critical thinking and clinical decision making to deliver optimal care outcomes for patients and/or their families.
Responsibilities:
● Nursing care for acute medical patient on interim care plan for investigation of presenting concern awaiting medical diagnosis with general patient cohort – seizure, TIA/Stroke, acute confusion, chest pain (non-cardiac), gastrointestinal bleed, syncope/collapse, DKA, PE, drug toxicity and overdose, eating disorder – refeeding syndrome
● Care across patient continuum – admission, assessment and care of deteriorating patient with MET escalation and MET participation, complex and rapid discharge within 48hrs presentation
● Provide leadership, direction and development to a team of RNs,ENs and AINs in the provision of patient holistic care to varied acute medical patient
● Administer medication; engage with doctors, manage documentation and report concerns to CN in charge –
IV infusion competence – iron infusion, magnesium infusion, blood and blood product experience
DKA competence – BGL and ketone monitoring with titration of insulin infusion – Statewide DKA protocol
● Telemetry – cardiac monitoring ● Provide activities of daily living (ADL) and support for patients with varying degrees of physical and cognitive capacity, ensuring they are treated with the utmost dignity and respect
● Engaging within a multi-disciplinary team and family members to ensure best practice and patient needs are met
● Palliative nursing care ensuring patient care and family support is provided – ARP and end-of-life care experience
● Critical thinking skills and assessing deterioration of a patient
● Precepting, mentoring and facilitating new staff and students within the ward and fellow new staff members ● Digital ieMR proficient – digital hospital program and experience with IT systems within healthcare including; RISKMAN, Trendcare, HBCIS, EDS
