  • I was given the responsibility to represent the company to train clinical pharmacist staff at a new HPS site, interstate, in Western Australia. I provided extensive training to all clinical pharmacist staff within the hospital on the use of best practices using our approved software, 'Clinpod', for hospital clinical rounds. I educated pharmacists at the hospital on how to use the software to ensure that all the key patient information is documented on the software in a time-efficient manner. During this training period, I supervised pharmacists on their wards, gave the pharmacists constructive feedback and answered all the further questions staff members had via Teams
  • Conducting clinical medication admissions, medicine reconciliations, medicine profiles, discharges.
  • Interacting with doctors, community pharmacists, nurses, and allied health professionals to provide the best patient-centred care possible
  • Handling sensitive situations well. Interacting with patients in many sensitive situations (particularly on the wards), and utilising time to best help with their medication management
  • Reviewing the charted medications prescribed by the doctors are correct, and contacting doctor(s) if the prescribed directions are unclear or if there are changes which may be needed
  • By using attention to detail, noticing discrepancies between what the patient was taking prior to admission compared with the medications the patient is currently charted by the doctor in the hospital. Flagging the discrepancies and contacting the hospital prescribing doctor, then liaising with the doctor to determine the most appropriate action to rectify the discrepancy.
  • Working with doctors, nurses and other pharmacy staff to ensure that patient care is always placed number one and at the centre of everything we do, and to do everything we can to ensure that patients are being provided the best possible medical treatment and that patients feel empowered to know how to effectively best take care of their medicines beyond the hospital.
  • For patients who are unable to take care of their own medicines, providing an updated patient medication list to their local preferred community pharmacy for continuity of care, so that patients can be provided with Medication Webster Packs (dosing aids), if the patient wishes to do so
  • Training new staff in Victoria in the dispensary on how to use Clinpod (for Clinical Pharmacy) and iPharmacy (for work in the dispensary)
