Working as the lead case manager for reputable home care service provider. This role included various tasks such as:
Conducting assessments of clients care needs, goals and preferences in order to coordinate support services.
Acting as the first point of contact and communicating professionally with older people and their families and collaborating with allied health professionals to deliver individualised and person centered care.
Managing the client's roster and schedule and attending to any last minute shift cancellations or replacements.
Supervising, training and mentoring new staff.
General office admin duties such as answering phone calls and responding to emails.
Greeting visitors to the office and supporting reception by managing bookings and handling incoming calls and emails enquiries.
Communicating with participants, families and liasing with external NDIS staff.
Managing participant records such as service agreements, care plans, and other documentation.
Reception duties such as scanning, faxing and filing of documents.
Ensuring that the office space is being kept organised and tidy.
Current first aid/CPR certificate
Experience using software such as Visual Care and Etools
