Adult Health Check Health program is a primary health care prevention screening tool that was developed from the Torres Model of Care, To manage Chronic Disease in the Torres Strait & NPA communities. Annual opportunistic screening which provides brief intervention to well population 15yrs - 54yrs+ it's aim is to identify and manage early signs of Chronic disease.

• To provide opportunistic, targeting and community Adult Health check screening throughout the District.
• Adult Health check Clinics, Community Screening.

• Refer clients to appropriate service and ensure follow up.
• Provide and deliver community feedback on Adult health check results.
• Maintain accurate documentation and adhere to legislative requirements.
• Uses the Primary clinical care manual,Chronic Disease guidelines and Best practice program.
• Update registers monthly and identifies clients who have not had an adult health check screen.
• Promote awareness and development of Adult Health check in the community in collaboration with the health promotion program through information recourses,major community events and the media.
