During my role with Plan and Grow I have supported children ranging in ages from 3-17 years of age in the home like environment. Most of Kids I supported coming from Department of community with very High NAT Level 4 or 5

Going to most of the house helping the client to clean the house and taking clients out to the community

• Assisted clients with daily living needs to maintain self-esteem and general wellness.
• Assisted clients in accessing available resources such as housing options, financial supports, and healthcare services.
• Transported clients to appointments, shopping venues and entertainment events according to determined schedule.
• Monitored clients' progress to adjust treatment plans accordingly.

• Meet with clients in home care to discuss their practical and emotional needs
• Regularly train family members in providing basic support techniques, as required
• Tend to the healthcare needs of each client including visiting the doctor, as needed
• Design care plans that best suit the clients' needs and assist in finding a job, if possible
• Assist with domestic tasks including shopping, cooking, cleaning and washing, as needed
• Consistently provide emotional support by talking to clients about their needs and listening to their concerns
• Regularly administer medication and provide strong assistance in managing finances, accessing community facilities, going shopping and meeting friends
• Engaged in helping clients to participate in leisure activities
• Key Achievements: Helped a client who had fallen and assisted him to hospital
• Stayed with them as long as possible and came back next day to check in and stayed and comforted the person

• Engaged in managing the daily activities of the team
• Regularly and precisely delegated a wide range of tasks to team members
• Conducted training of team members in order to maximise their potential, as needed
