  • In my role of National Program Manager HCP I was responsible for providing leadership to the HCP team across Focus Care’s operations to ensure adherence to the Home Care Packages (HCP) Program Guidelines and the Aged Care Quality Standards and User Rights Principles. Providing advice and support to Focus Care’s Organisational Governance Committee to ensure Policies and operational procedures maintain compliance with the HCP Program and opportunities for continuous improvement are identified and implemented, ensuring best practice Case Management to HCP Clients in the co-design of Support Plans and actively maintaining a broad network of relevant service providers to ensure consistency and relevancy of care and support options available to clients

  • Maintaining knowledge of current HCP funding guidelines and program requirements as well as the Support at Home Program Reforms and report on key issues to relevant Focus Care Governance Committees
  • Oversee intake and discharge processes for all HCP clients
  • Oversee leads, referral source and analysis of conversions across all regions
  • Ensure HCP service delivery is managed in accordance with the relevant quality standards and evidence based best practice community aged care support
