• Coordinated discharge planning and multidisciplinary case reviews.

• Provided direct patient care, including support with daily activities such as bathing, dressing, and mobility.
• Assisted with safe patient positioning, turning, and transfers to promote comfort and prevent pressure injuries.
• Helped maintain a clean and safe environment by following strict infection control practices.
• Supported patients with meals, including feeding assistance and monitoring dietary intake.
