• Whilst working as a Assistant in Nursing, I then was promoted to team leader, the responsibilities include:
• Perform comprehensive assessments to identify the resident’s needs
• Develop individualized care plans based on the needs of the resident and co-ordinate with the RN, Family members and the allied health including the physiotherapist, social workers and community resources
• Implement the care plan
• Ongoing monitoring and evaluation of the resident’s needs and make adjustments to the care plans to meet their unmet needs
