Assisting in developing, implementing and evaluating plans that enable people we support to set and achieve personal goals
Resident contact and interaction including attending to their personal care and undertaking general domestic duties
Showing empathy and compassion towards residents and display genuine care and concern for resident well-being
Encouraging positive and socially appropriate behavior, and effectively manage challenging behaviours
Working as a Support Worker managing and mediating complex changing behaviours
Supporting people to access a quality life with dignity, respect and social inclusion in the least restrictive environment
Supporting client well-being assisting with household activities and preparing meals and social, recreational and community experiences that enhance quality of life based on support plan goals
Documenting accurate and complete records of work activities
Developing planned activities in line with departmental policies and guidelines
Liaising with management, service providers and stakeholders
Ensuring continual duty of care is upheld for clients, self and others
Supporting and empowering clients to live their life with autonomy and independence
Manual handling tasks including but not limited to transfers and mobility
Direct personal care support and overall domestics/household duties
Administration of medication
Supports client physical and emotional health and wellbeing in an appropriate manner
Promotes capacity building, restorative care and social inclusion to maintain and promote independence and autonomy of the disabled
Records and maintains client records as required
Attends team meetings and training activities as required
