  • Conducting face-to-face interviews with supported employees to perform assessments such as health check-ins, CANSAS (Psychosocial need assessments), NDIS service agreements, Individual Employment Plans, and Programs of Support (POS) by using recovery-focused, strength-based language and a person-led approach to assess needs and determine the most suitable services for them.
  • Encouraging and empowering supported employees or individuals with complex mental health issues to actively participate in their assessments by providing them with more choice and control over the process. Additionally, assisting them in developing and setting realistic goals using the SMART goal-setting technique.
  • Recording and maintaining accurate notes in a person's profile in the Client Information Management System/Service Record System (CIMS/SRS).
  • Tracking supported employees' progress to adjust the program of support and Individual employment plans accordingly.
  • Collaborating with other professionals such as mental health workers, site manager, and agency supervisors to plan and coordinate care for service users.
