• Currently work as an Agency RN or ANUM as required by the Residential Aged Care Facility
• Person Centered Care and Assessment for each Resident in Residential Aged Care - ensuring resident safety, appropriate personal care, management of complex chronic disease
• Document all Assessments, Care Plans and Daily Reports on the Hospital Information System.
• I am familiar with MANAD as used by Dhelkaya Health and Maldon Hospital
• Communicate with the Resident and Families in a kind, empathetic manner – listening and providing honest answers to the Resident and their Family
• Referring family concerns to NUM, ANUM and the Interdisciplinary Team ensuring excellent communication to enable coordinated nursing
• Palliative Care Management as per Policy and Protocols
• Provide support, direction and education to PCWs, ENs, Student Nurses and Graduate Nurses
• As the ANUM or RN, I work with the nurses, listening to their concerns, attending to Resident and Nursing concerns, ensuring the team is working collaboratively for the benefit of the Resident
• Consult with RNs, NUM and AHM regarding concerns
• Liaise with GPs and Specialists, providing feedback to the Medical Team, advocate on behalf of the Residents/Families, following up on medical orders/medication changes
• Work within my scope of practice as established by AHPRA, Nursing and Midwifery Board of Australia, Federal Endorsement of Aged Care Standards of Care, Hospital Policy and Protocols as on PROMPT
• I understand the AN-ACC Assessment for Residential Aged Care.

• Admission, monitoring, discharge planning for TCP Clients
• Liaised with TCP Multidisciplinary Team, Medical Specialists, Gerontologists, GPs, Community Health Care Providers, Home Care Package Providers, Private and Public Hospital Discharge Planners and Families - regarding client’s current care and ongoing discharge planning
• Maintained accurate concise Client Documentation and Client Centered Care Plans.
• Managed patient caseloads effectively, ensuring timely follow-up and appropriate interventions.

• Case management for fifty clients ensuring that each client had appropriate community care services to enable clients to remain safely at home for as long as possible
• Assisted families and clients to navigate My Aged Care, Service Providers, GP, Practice Nurses, Community Nurses, Carers Gateway
• Timely record of each Client contact, Client communication, Client budgets and provision of Client Centered Care Plans.

Medical assessment of donors prior to donation of Whole Blood and Plasma

• Canulation/Setting up/monitoring Apheresis and Whole Blood equipment during Plasma or Whole Blood donation
• Medical management of donor adverse reactions from blood/plasma donation and liaison with Lifeblood Medical Team and if required Ambulance Paramedics regarding Donor adverse events
• Session Leader when required to manage Lifeblood Clinic, provide guidance to Lifeblood Team re Donor complications, TGA and Lifeblood Policy and Procedures, monitor and maintain OH&S protocols for donors and staff
• Session Leader ensured Plasma and Whole Blood stored, packaged and transferred correctly for safety of blood product according to TGA guidelines.

• Liaise with Hospital Medical and Allied Health staff to identify and assess clients with Chronic and Complex Health from the Acute Ward, Rehabilitation Ward and Community for admission to the Hospital Admission Risk Program
• Prioritised, documented, gained consent from accepted HARP clients and then discussed with the HARP Care Coordinators – providing information regarding the medical and social history of each client
• Communicate with family members providing information re HARP and the ongoing care requirements for the client on discharge from the hospital.
