With Care Connect I was the only Care Manager based on the QLD Sunshine Coast following their expansion into the area.
As a Care Manager I completed consultations with prospective clients, explored their care needs, and discussed how we could assist them in remaining active and safe within their own home in their own home.
Once the client had chosen Care Connect as their provider, I worked in partnership with them to develop a care plan in line with their available HCP budget.
I supported 50 HCP clients with ongoing monitoring of their services and supports through regular touch-point phone calls, home visits, and reviews.
My role involved networking with other organisations to build the Care Connect brand in the area. I was required to travel to the main Brisbane office once a month for team meetings.
In my previous roles, I have used a variety of client relationship management systems, including Alayacare, Sugar, and Salesforce.
I moved on from this role to open the Home Instead (now Dovida) Franchise on the NSW Mid North Coast.
