Area of Practice: Chemotherapy, Dispensary, North Ward (Med/Surg), and South Ward (Rehabilitation Wards).

Role:

Through my role as a Grade 2, Year 3 Pharmacist, I have been able to develop and refine my skills in clinical pharmacy services, while expanding my leadership responsibilities. One of my key achievements has been developing guidelines and procedures to enhance clinical pharmacy services, to ensure consistent, high-quality patient care.

I’ve also delivered specialised medication services to oncology patients, including developing protocols for chemotherapy agents, to ensure that our oncology service is standardised and evidence-based.

My strong ability to collaborate with physicians and nurses has helped me contribute meaningfully to the ward rounds and the daily functioning of the pharmacy department. As part of my day-to-day responsibilities, I consistently review medication orders for accuracy by assessing pathology results, potential drug interactions, contraindications, and allergies.

In addition to patient-centered care, I have actively participated in formulary management activities, evaluating the inclusion or exclusion of new and existing medications based on cost-effectiveness, and clinical benefits.

Additional Activities:

• Led initiatives to optimise therapeutic outcomes by educating healthcare providers on new monitoring systems (Guidance) and connecting it with current guidelines.
• Developed an auditing tool for the auditing of Schedule 8 Registers.
• Undertook the Foundational Seminar in Oncology to improve my knowledge of cytotoxic agents and immunotherapy, to be able to clinically verify and dispense chemotherapy medications.

Area of Practice: Complex Care, Dispensary, Medical, Surgical, and Rehabilitation Wards.

Role: Through my demonstrated competence on the wards, I earned a promotion to Grade 2 pharmacist. This recognition led to my rotation into the role of Complex Care Pharmacist. In this role, I followed up with difficult and specialized patients who required more advanced management and clinical review. I successfully educated patients on proper medication use and adherence, with the aim of improving health outcomes, and reducing hospital readmissions. I navigated the intricate patient care of my Complex Care patients, played an integral role in providing expert guidance on appropriate drug therapy interventions, and collaborated closely with multidisciplinary teams to optimize my patients' therapeutic outcomes. I enjoyed building good rapport with my patients and developing strong working relationships with my colleagues. My promotion and transition into this role underscored not only my proficiency in clinical practice, but also my commitment to delivering exceptional care in challenging healthcare settings.

Additional activities:

• Led patient-focused initiatives to enhance the overall patient experience within the healthcare system, resulting in higher levels of satisfaction and trust among patients. I collaborated with the Complex Care's Respiratory Nurse to deliver patient education at Njernda Aboriginal Corporation on the general medication management of COPD and asthma. This education was extremely appreciated, and Complex Care was asked to deliver another education session on cardiac medications.

Role: As a fully qualified pharmacist, I consolidated my skills through hands-on experience and ongoing professional development. Building upon the foundation laid during my intern year, I refined my clinical thinking by continually analysing medication regimens and using

evidence-based protocols and guidelines to optimise outcomes. Attention to detail remained a priority as I ensured accuracy in medication dispensing and methadone management. Engaging with patients to obtain comprehensive medication histories allowed me to tailor pharmaceutical care plans to individual needs effectively. Moreover, encountering different clinical scenarios provided me with opportunities to further fine-tune my problem-solving abilities, enabling me to address complex medication-related issues with confidence and expertise. Through a commitment to continuous learning and practice, I solidified my role as a competent and compassionate healthcare professional dedicated to delivering high-quality patient care.

Additional Activities:

• Oversaw inventory management processes within the pharmacy department, ensuring accurate tracking of medications for regulatory compliance purposes.
• Delivered outpatient education to patient groups and nurses, fostering trust and communication with the community. I created and delivered education to nursing staff on constipation and the use of laxatives. I also delivered prepared talks on medications used in pulmonary and cardiac rehabilitation.
• Supervised and mentored intern pharmacists and students on the wards and in dispensary.

Role: During my intern year as a pharmacist, I developed crucial skills. Critical thinking advanced working through complex medication regimens under the guidance of my supervising pharmacist. Attention to detail improved ensuring accuracy in dispensing, documentation and interpreting guidelines. Communicating with patients to obtain medication histories deepened my understanding of patients and their conditions. Problem-solving was honed through diverse clinical scenarios and task prioritisation, helping me address urgent medication-related issues promptly.

Role: In my final years of university, I acquired a second pharmacy student role during which I was able to fine tune my patient counseling and communication skills. I learnt to convey information clearly and empathetically, appropriately guide individuals on medication usage and address any concerns they may have, and fostered positive interactions with patients and healthcare professionals alike. I ensured the stores inventory was accurate by verifying orders and maintaining detailed records. I practised my attention to detail by accurately and safely packing Webster Packs and I collaborated with pharmacy colleagues to fulfill prescriptions and optimise operational efficiency.

• Contributed to increased sales by offering advice on over-the-counter medications and products.

Role: I started working at Portland Amcal Pharmacy from the age of 15. I started at the front of store which gave me courage to develop communication skills, team work and work ethic. I was then trained to work in the dispensary which ignited my passion for pharmacy. I enjoyed solving medication-related issues and providing comprehensive patient counselling, to empower patients in managing their health effectively. I also enjoyed offering advice on over-the-counter medications and products.
