Professional Practice
• Ensure that care is delivered in accordance with the philosophy, policies, and procedures of Estia Health, and complies with all relevant local, state, and federal legislative requirements, and related principles.
• Lead and participate in the delivery of a high standard of person-centred care.
• Provide a high standard of ongoing clinical management and performs nursing interventions in
accordance with recognised standards of practice.
Assist and educate staff to complete comprehensive, individual assessments, and care plans for new and existing residents, and evaluate care in line with Estia Health policy.
• Ensure the social, emotional spiritual and recreational needs of all the residents are a high priority in
the care regime and that all care staff are supported in working towards these needs.
• Practices in a way that acknowledges the dignity, culture, values beliefs, and rights of individual
residents are respected.
Nursing and Planning for Care
• Conducting initial comprehensive clinical assessments for input to the care services plan for the
resident, carried out:
o in line with the resident’s needs, goals and preferences; and
o if required in consultation with other appropriate health practitioners, appropriate allied health
professionals.
• Ongoing, regular, comprehensive clinical assessment of the resident, including identifying and responding appropriately to changes or deterioration in function, behavior, condition, or risk, carried out: o in line with the residents’ needs, goals, and preferences; o if required, in consultation with other appropriately registered health practitioners and/or appropriate allied health professionals.
• Other nursing services.
• Ensuring that families and/or authorised representatives are included in the care planning process and in any decisions affecting the resident.
• Provide direct supervision of health professionals’ activity (including allied health and medical
practitioners) to support the management of clinical care delivery which includes the assessment,
planning, implementation, and evaluation of care for each resident.
• Ensuring that all clinical documentation is completed accurately and in a timely manner in accordance with Estia Health policies and procedures. This includes care plans and clinical assessments.
Supports General Access to Medical and Allied Health Services
• Makes arrangements for registered health practitioners to visit the residents for any necessary
registered health practitioner appointments.
• Makes arrangements for the residents to attend any necessary registered health practitioner
appointments.
• If required, makes arrangements for allied health professionals to visit the resident, or for the resident to visit an allied health professional.
• If required, provision of audio-visual equipment for use with telehealth appointments.
• arranges for an ambulance in emergency situations.
Medication Management:
• Implementation of a safe and efficient system to manage prescribing, procuring, dispensing, supplying, packaging, storing, and administering both prescription and over-the-counter medicines.
• Administration and monitoring of the effects of medication (via all routes, including injections), including supervision and physical assistance with taking both prescription and over-the-counter medication, • Reviewing the appropriateness of medications as needed. Dementia and Cognition Management If a resident has dementia or other cognitive impairments:
• Ensure the development of an individual therapy and support program designed and carried out to: prevent or manage a particular condition or behaviour; enhance the resident’s quality of life; and enhance care for the resident.
• Provide and ensure ongoing support (including specific encouragement) to motivate or enable the resident to take part in the general activities of the residential care home (where appropriate).
