· Provide Carinbundi’s Clients and representatives with information, support, and practical assistance to identify Client’s goals and aspirations; and inform/assist with decision making on supports.
· Where relevant, liaise with Health and other Support Providers to gather and verify relevant information to facilitate plan development.
· Work with Clients and their families/carers to identify options to achieve their plan outcomes, drawing on informal, mainstream, and community supports; and reasonable and necessary supports that are to be funded by NDIS.
· Conduct support needs and risk assessments, interpreting sometimes complex information about functional limitations, personal factors, and context, and how they impact on daily life; and consistently apply functional assessment tools. and authorise specialist assessments where required.
· Participate in internal Quality Assurance processes and Continuous Improvement processes – Client and Location Quality Audits.
· Updating SIL ISP’s, Budgets, Financial Management Plans, Risk Assessments, Goal Reviews.
· Work individually, and collaboratively, to maintain a high quality professional service for all Clients, families and/or carers.
· Ensure the implementation of each Client’s documented needs, interests, and care requirements - as detailed in the personal file.
· Ensure confidentiality in all matters relating to the clients, families, and service.
· Establish positive relationships with families, or significant others, in the person’s life.
· Provide progress summary’s for community clients if required – goal reviews.
· Ensure SIL clients have rooming agreements in place and update when required.

Financial · Update budgets and financial plans for SIL.
· Manage individual commitments and obligations including scheduling, prioritising, and managing work flows against departmental goals.
