-Supporting at front desk of Foster House men's accommodation, responding to the inquiries via phones, email and walk-in and updating case notes in timely manner.
-Shadowing Case managers and Intake Assessment Workers during the client meeting and while conducting need based assessments.
-Supporting Case Manager to set up client profile on the CIMS database, writing case notes and creating case plans for basic brokerage requests.
-Supporting Case Manager with phone calls and referrals to the other services.
-Working alongside the support workers with organising events, decorating, doing welfare checks, assisting in dinning area, providing basic support to the residents and engaging in the activities with them.
- Attending community hubs; Aboriginal wish hub and City salvos central community hub with case managers and gaining experience working with clients experiencing homelessness closely.
-Assisting residents with daily living activities; bathing, grooming, dressing and engaging with them doing recreational activities in spare time.
-Helping patients with mobility, assisting them with feeding and providing assistance with meals as needed.
-Working in a multi-disciplinary teams to provide client-centred support and services to meet residents need.
-Working closely with residents and providing emotional support and companionship, especially to those who may be feeling anxious. distressed and lonely.
-Monitoring and reporting changes to the Registered Nurse in-charge and documenting notes in the system.
