Duties include providing support to 2 residential homes (360 beds) in ANACC, requesting reclassifications reviews, data reviews, liaising with ANACC assessors, admission teams, assisting with admission ANACC predictions. Provide staff education regarding ANACC and clinical documentation changes, prepare weekly reports, conduct weekly ANACC meetings with leading clinical staff, provide support to staff, quality reviews of care plans, gap analysis and risk management. Monitoring of clinical changes in clients identifying reassessments. Completing ongoing assessments of clients and their care needs, reviewing and adjusting relevant care plans
