completed clinical rotation in comprehensive stroke and aged care rehabilitation unit (SRU). roles and responsibilities while working on this ward included;
- Working collaboratively in the assessment and management of a patient load of four acute medical, stroke or rehabilitative patients
- Following hospital policies procedures and standards when providing nursing assessment and interventions to acute stroke patients for the early recognition of complications which increase patient mortality and morbidity.
- Attending and accessing relevant professional development opportunities and completing mandatory hospital competencies.
- Providing patient education, resources and referring to relevant allied health care professionals.
- Experienced in dysphagia, safe swallow practices and modified diet and fluids.
- Insertion and management of Enteral tubes.
- Bladder management, including insertion and management of Indwelling and supra pubic catheters and Intermittent catheter regimes.
- Patient goal and outcome planning and evaluation.
- Coordination and integration with the multidisciplinary health care team.
- Assisted patients to complete activities of daily living, facilitating increased independence while maintaining duty of care and patient safety.
Current clinical rotation in general medical ward. Job roles include;
- Managing the clinical oversight of up to six patients or team nursing up to ten patients.
- Evaluating patient care needs, prioritizing treatment and ensuring patient flow.
- Establishing and maintaining a therapeutic relationship with patients and communicating effectively using a variety of channels.
- Advocating and facilitating the rights of the individual.
- Experience with various medical conditions including diabetes, chronic obstructive pulmonary disease, congestive cardiac failure, substance abuse, sepsis, cancer, communicable and neurological diseases.
- Adhering to occupational health care guidelines and taking action to avoid, minimize or eliminate hazards, this includes documenting and reporting incidents and hazards.
- Wound assessment and management. Risk assessment and management for the prevention and treatment of pressure injuries and skin tears.
- Respiratory assessment and management. Oxygen therapy devices and non invasive ventilation.
- Intravenous medication, fluid and antibiotic administration through IVC and PICC lines.
- Early recognition and assessment of the deteriorating patient using the Airway, Breathing, Circulation, Disability, Exposure (ABCDE) pathway.
- Supervision, training and task delegation of undergraduate, enrolled and assistant nurses.
- Use of computer systems such as Isoft and DuraPage. Correct documentation and recording of patient care and interventions.
