  • Secondment to State Health Incident Coordination Centre, Public Health Operations to stand up a Model of Care prior to the opening of WA borders for the response for vulnerable population groups with complex high-risk COVID19 cases and close contacts in the community. COVID CAT staff assisted vulnerable population groups to safely isolate during the COVID - 19 pandemics.
  • COVID CAT program was modeled on the COVID Surge Plan developed in 2020 for the vulnerable population group. The program was implemented with a workforce of 20 x staff as a WA statewide service on the 11th February 2022.
  • Developed and implemented COVID CAT project to deliver culturally appropriate support with resource packages, community linkages and referral pathways to Primary and Allied Health service providers, individual case coordination, follow up and advocacy for WA community members isolating as a positive case of COVID – 19.
  • Provide support in State Welfare Coordination Centre (SWICC) hotels to clients with complex needs who require additional support whilst isolating to reduce the risk of avoidable hospital presentations and non-compliance with isolation.
  • COVID CAT program consists of a multi-disciplinary workforce of Care Coordinators, Registered Nurses, Allied Health Professional, Project Officer and Aboriginal Health Liaison Officers.
  • Specialized case management and support has been provided to 5379 clients including assistance with referral pathways for social and emotional wellbeing, mental health and psycho-social support, drug and alcohol withdrawal and access to schedule 8 OST, domestic violence, disability, medications, homelessness and chronic conditions.
  • Establish and maintain participation, partnership and collaborations with key stakeholders, individuals, groups and communities
  • Implemented a delivery model to provide a collaborative approach with stakeholders and subject matter experts such as: • Aboriginal Health Directorate • COVID Care at Home • Public Health Operations (PHops) • Department of Premier & Cabinet • Department of Communities • Western Australia Country Health Service (WACHS) • Patient Assistance Transport Scheme (PATS) • State Health Incident Control Centre (SHICC), State Welfare Incident Control Centre (SWICC) • Other external stakeholders.
