Mental Health Care Coordinator

  • Act as the first point of contact for all new primary & secondary care referrals. This is includes acting as switchboard for service users & professionals i.e psychology department.
  • Answer & respond to 50+ telephone calls, emails & voicemails from service users, family members & other professionals I.E Crisis team & GP's on a daily basis.
  • General admin tasks - attend, record & distribute minutes from MDT's, organise post, greet visitors face-to-face & via the telephone, manage diaries, schedule meetings & book rooms.
  • Screening of paper & electronic referrals from GP's & other healthcare services - Evaluate patients to identify risk, assess suitability for services based on referral presentation, level of intervention required, mental health history etc & implement treatment plans.
  • Direct referrals to appropriate department, internal or external i.e Cognitive Behavioural Therapy.
  • Established rapport and maintained knowledge of local community services and outreach programmes.
  • Maintain record of all activities carried out - outbound calls, calls received, email correspondence, voicemails, notes from GP's etc.
  • Process formal requests from users such as Subject Access Requests, medication queries & complaints.
  • Process patient appointment bookings & signpost patients to other relevant services.
  • Assist in supervising staff & new starters– Provide system access permissions, email access & liaise with IT & HR to ensure a smooth starting process.
  • Provide performance reviews, training & support to Graduate Mental Health Workers.
  • Provide support to the clinicians - assist with booking appointments, typing up notes following consultation & system guidance support.
  • Record & collate patient data for a daily review meeting to assist with triage function, to avoid breaches, and monitor caseloads to ensure it does not exceed safe working limit.
