Job Duties

    DIABETIC EDUCATOR DUTIES


  • Provide diabetic education in consultation with the medical team and nurse practitioner according to guidelines and procedures.,
  • Educate Patient and their families with Blood Glucose monitoring and Management.
  • Seeing patients with newly diagnosed type 1, type 2 and GDM.
  • Education patients and their families with insulin administration, storage and safe disposal.
  • Assess and re-educate patient with recurrent Hyperglycemias and Hypoglycaemia’s.
  • Organise dietician for implementing a nutritional care plan for patient specific, establishing a follow up system post discharge,
  • Supporting and assisting their families during hospital stay.
  • Providing booklets and pamphlets for healthy diet, steps for Insulin administration, BGL monitoring device, sick day management plan, Hypo management plan and contact details,
  • Referring them for follow up with Endocrinologist/Nurse Practitioner as required.


    REGISTRERED NURSE DUTIES


  • Collaborate well with multidisciplinary team to assess patient data and allocate patient care plans.
  • Able to respond to patient emergencies with out delay and provide correct intervention.
  • Able to administer medications and treatments according to manage patient's condition.
  • Able to utilise evidence-based practice to assess and deliver patient care.
  • Educate patients and families to increase their knowledge and understanding and provide best possible ways for a better outcome.
  • Maintain proper and up to date comprehensive patient records in line with regulatory standards.
  • Demonstrate knowledge of using iEMR and proper equipment use.
  • Maintain patient confidentiality by protecting unauthorized access to personal information.
  • Assist patients with general patient needs and care
  • Able to manage verbally abusive and aggressive patients.
  • Manual handling, ADL Assistance,Wound dressing, Promoting and maintaining good hygienic practices.
  • Intake and Discharge
  • Maintain Professionalism and develop positive relationship with patients/family, co-workers, allied health team, medical team and external people.
  • Promote team work by supporting and Helping colleagues. Supervising and directing EEN's and Students.
