I have worked as a Care Coordinator, with responsibilities including care planning, admissions, and assistance with AN-ACC. I have undergone training in wound management and have experience of working as a key personnel in a management role.

• Providing high standard of service that supports and assists residents to achieve maximum independence while adhering to their care plan.
• Assisting residents with their activities of daily living with consideration of individual preferences.
• Undertaking all duties in accordance with company’s quality management system, policies and procedures.
• Monitoring the condition of residents and taking clinical actions accordingly.
• Maintaining a positive, caring, patient and friendly demeanour at all times.
• New resident admissions, formulating, reviewing and updating care plans.
• One on One assist to the highly vulnerable patients.
