This role includes implementing therapy plans designed by a range of allied health professionals (OT, SP and PT) for individuals living with disabilities to support the development of a series of habits within their routine
I have gained experience, supporting participants in morning/afterschool routine, self-care tasks (including showering), meal preparation and assisting with accessing travel/transport in the community.

Demonstrate initiative to ask supervising practitioner for more strategies/resources to support participant
Experience in conducting neuro-based assessments with a range of presentations including changes to cognition, spatial neglect, and upper limb weakness
Experience with assessments including MoCA, Oxford Cognitive Screen, RUDAS, Motor Assessment Scale (MAS)
Developed skills in upper limb rehabilitation
Experience in discharge planning and completing discharge summaries
Collaboration with multidisciplinary team to deliver client centered practice
