Sophie has demonstrated comprehensive documentation across a range of modalities including chart entries, NDIS documents and case conference notes. She is able to proactively liaise with the MDT and identify options for follow up therapy.”
Ability to manage upwards of 8 patients in an inpatient and/or outpatient setting with the ability to provide appropriate education, treatment and care along with the following achievements:
- Managing a mix of inpatient and outpatient caseload in timely fashion
- Timely referrals to appropriate colleagues within the MDT to optimise patient care and outcomes
- Ability to identify and refer to appropriate follow up care within the community setting
- Appropriate and effective discharge planning
Ability to communicate effectively and efficiently with other disciplines throughout clinical placements via numerous communication methods to manage patients:
- Communicating efficiently with surgeons and doctors regarding post operative/procedures protocols - Complex discharge planning
- Managing complex patients
- Represent the discipline in MDT meetings discussing plans
Accomplished successful treatment outcomes in acute stage neurological, stroke, and spinal patients including:
- Acute Tracheostomy care, including suctioning, educating self-cleaning and management
- Progressive intensive physiotherapy care in acute stroke and brain injury patients
