• Responsible for the allocation of client visits including new referrals, coordination of resources, clinical care delivery, and monitoring the delivery of direct care and participation in multidisciplinary meetings.
• Providing leadership and supervision to promote ongoing development of our direct care teams. Ensuring compliance with policies and procedures enabling the delivery of high quality health care in the community.

• Patient assessment and monitoring of medical conditions. Monitoring vital signs, symptoms and responses to treatment
• Care coordination with health care professionals from the SCUH HITH team to develop and implement care plan including arranging necessary equipment and supplies for home care.
• Medication administration including intravenous therapies, antibiotics, and other prescribed treatments.
• Venepuncture, IV Cannulation, Wound care including VAC (negative pressure wound therapy) & PICO dressings.
• Documentation and reporting, maintaining accurate and detailed patient records, including assessments, treatments and progress notes. Reporting changes in patients condition to the supervising physician and health care team
• Patient education
