I am a well-presented and very personable individual with experience in patient care in sub-acute specilised aged care setting with MNHHS at Brighton health campus and as clozapine coordinator in community and mental health with MSHHS, I possess range of abilities which enables me to successfully help the clients in managing their health by establishing therapeutic relationship with the patients comfortable in interacting with people from all backgrounds and my aim as a nurse is to provide support to the patients in culturally safe manner help them in their recovery. Clozapine coordinator, Woolloongabba Community and mental Health, Jan 30, 2023 - current, Running clozapine clinics which includes vital signs observation and recording on CIMHA, following blood test results of clients for the clinic and record them., Liaise with other members of multi-disciplinary team for appointments for clozapine consumers, liaise with community pharmacies for medication dispensation by community pharmacies., Educating the clients about medications, safe administration and their side effects and how to mitigate them, smoking cessation planning and educating clients on smoking cessation pathway., Working along with case managers of clients to make sure they get their medication on time which includes home and hospital visits., Do regular home visits with other case managers in the community setting to administer depot injections and medications., Educate them on clinical trials of new medications and guide them how to access the information and whom to contact., First point of contact for all doctor only consumers in clozapine clinic and help them in their need and do appropriate referral for them if they require additional support or short time case management., Help other community and Acute care teams in assessing new referrals and doing MSE and Risk screening.
