Summary

Therapeutic professional in Case Management, providing support to Children and Young People (CYP), families, significant others and relevant stakeholders within an Intensive Therapeutic Care (ITC) program in Out Of Home Care (OOHC). The CYP within ITC have experienced complex trauma backgrounds, and as such, have complex and challenging needs. Service delivery is child centred based on a therapeutic framework focusing on strengths based intervention and support.

  • Managed a case load of Children and Young People in accordance with relevant legislative and OOHC standard, funding requirements and practice guidelines.
  • Evaluated program outcomes regularly, identifying areas of improvement and implementing changes to enhance service delivery for all clients.
  • Ensured compliance with all federal and state regulations related to case management practices by maintaining meticulous records and staying abreast of relevant laws and policies.

  • Intensive collaboration with care teams of the children and young people on a regular basis (i.e. Development of routines, programs, protocols, strategies, modelling positive and supportive interactions, the development, review and implementation of restrictive practices etc).
  • Development and implementation (with all relevant stakeholders) of case plans, leaving care plans, after care plans and cultural support plans of the children and young people. Through this it is then essential to have monitored client progress carefully over time through reviews, adjusting the plans as needed based on evolving needs or changing circumstances.
  • Maintained up-to-date knowledge of community resources, connecting clients with appropriate supports to meet their unique needs.
  • Proactively addressed situations of direct crisis by utilizing de-escalation techniques and crisis intervention strategies through modelling the training and learning of Therapeutic Crisis Intervention.
  • Maintain and hold professional relationships with all stakeholders (i.e. CYP, colleagues, external agencies, government departments, interagencies, allied health, birth family and significant others of CYP). These relationships are respectful whilst maintaining confidentiality, privacy, appropriate boundaries and supportive to provide a safe environment and consistency for the CYP.
  • Mentored new case managers and colleagues by sharing best practices and providing guidance in challenging circumstances, fostering a culture of continuous learning within the team.

  • A focus on creating a home like environment for CYP who live in the OOHC ITCH.
  • Ensure the coordination of all services at the placement, ensuring the needs of the CYP are met in consultation with the relevant stakeholders.
  • Provide support, leadership and guidance to a team of Therapeutic Support Workers who report directly to the Therapeutic Support Manager.
  • Responsible for the direct management of TSW's (included but not limited to: Regular supervisions and performance reviews, complaints and allegation management, inductions, training, facilitating ongoing meetings, rostering, payroll and ensuring that the TYW's are aligned with the therapeutic framework and following all legislative requirements and OOHC standards).
  • Responsible for coordinating and monitoring the implementation of case plans in collaboration with the care team and CYP.
  • Coordinating the ongoing completion of Life Story Work for the CYP.
  • Meeting all WHS requirements.
  • Responsible for the day to day management of the ITCH, creating a home like environment reflecting the NSW OOHC standard, individual care needs, values, strengths, abilities, culture, personalities and aspirations of the CPY who reside within the placement.
