My responsibilities include Baxter peritoneal dialysis, APD, and PD; providing acute care for patients with renal transplant complications; maintaining and troubleshooting renal access such as AVG, AVF, Permacaths, IJVC, and FVC. I am involved in the assessment, planning, implementation, and evaluation of patient needs. I manage patient admissions, transfers, and discharges in collaboration with bed managers. I provide leadership, allocate staff, support staff decision-making, and refer to appropriate members of the multidisciplinary team. As a clinical nurse in the renal inpatient ward, I have managed the people handling portfolio, conducted training, and completed annual assessments for all staff, uploading these records into the online learning system. I am skilled in accessing arterial venous fistulas and grafts, and trained in ultrasound-guided cannulation of AVF/AVG. I am adept at managing patient fluid assessment and ultrafiltration before and after treatment. My expertise includes assessing, planning, implementing, and evaluating patient needs. I also educate and supervise new staff. I handle maintenance and troubleshooting for renal access, such as AVG, AVF, Permacaths, IJVC, and FVC. During my time in the dialysis unit, I served as a trainer for people and manual handling, responsible for the training and annual assessment of staff. In this role, I initiated a three-month trial to position all hemodialysis machines on the left side of the patient's chair/bed to prevent staff injuries, as the unit frequently moved machines between patient appointments. The trial was successful before I transitioned to the renal inpatient ward.

Provided quality care management for patients with chronic kidney disease and end-stage renal disease, who also faced multiple co-morbidities. Instructed patients and caregivers on proper wound care, discharge planning, safe medication practices, and disease management. Monitored patients' responses to medications and intravenous therapies. Promptly addressed any situations that affected the safety and security of the unit, implementing crisis prevention measures to control and de-escalate incidents. Recorded all treatments provided, medications and IVs given, discharge directions, and follow-up care instructions using the IEMER System.

I have conducted Fresenius peritoneal dialysis, APD, and PD; provided post-operative care after renal transplants; administered acute care for renal transplant complications; managed diabetes care, including insulin infusions; and delivered acute care to hemodialysis patients. When necessary, I have served as a Charge Nurse, leading other staff members. Duties encompassed assessing, planning, implementing, and evaluating patient care, in addition to maintaining and troubleshooting renal access.
