    · Created and implemented a new process for seeking supported independent living funding for multiple clients who were inadequately funded. This was to ensure clients received the budget required to meet their care needs and keep them safe and secure in their homes. I was able to deliver the evidence required to the NDIS so they could make more informed decisions on funding these clients more appropriately. This information was then used to develop and deliver a "lunch and learn session" which I facilitated for the Capacity building program, to explain how this was developed, and what tools had been developed along the way.

Young Onset Dementia: A specialist team

   

A composed of Allied health therapists, PBS practitioners, an engagement   coordinator and myself as support coordinator. This group has been formed to provide   specialised services for clients with Young Onset Dementia. The goal is to   support families in assisting their loved ones during this challenging time and to connect them with the necessary services.  The team has been working on Processes and support strategies to assist the client and their families through this difficult time
