Community Access Responsibilities:

• Assess and plan personalized community engagement for individuals with disabilities
• Arrange and assist with transportation to community activities
• Promote inclusion in various community events and programs
• Provide support during outings, ensuring safety and enjoyment
• Advocate for individuals' rights and needs within the community
• Support skill development for meaningful community participation
• Document activities, progress, and contribute to ongoing planning

Home Visit Responsibilities:

•Assist with personal care and daily living activities at the individual's home.

• Administer medications and monitor health as per prescribed plans.

• Plan and prepare nutritious meals based on dietary requirements.

• Help with light housekeeping and ensure a safe living environment.

• Provide companionship and emotional support during home visits.

• Collaborate with families for a holistic approach to care.

• Keep accurate records of home visits, observations, and activities.
