One of the residents at my workplace doesn't speak English at all. Aged Care Facility 4 weeks (160 hours) October-November 2022
· During my aged care placement, I often noticed signs of withdrawal in a resident who was usually isolated. I initiated a gentle conversation, thinking I could bring back good memories from the past. I also collaborated with the senior carers and the Diversional Therapist (DT) on duty to include personalized activities such as bingo, which the resident had mentioned that they enjoyed (in their care plan), which resulted in the resident becoming more socially engaged and improving their mood. I received great feedback from staff for noticing the resident and taking the initiative in a fast-paced morning shift, where everything was so busy that things like this usually went unnoticed.

Subiaco (Ortho ward) 5 weeks (200 hours) October-December 2024
· During an afternoon shift in ward 51, I received a postoperative patient who had a major hip replacement and was very reluctant to mobilize after the surgery. After conversing with the patient, I found out that the patient was in a lot of pain and was scared to ambulate. I escalated this concern to my buddy RN. We then administered pain medication that was charted while utilizing non-pharmacological comfort techniques such as ice packs as well. This resulted in effective pain management, and we were able to mobilize the patient timely after the surgery.

Cardiothoracic and Vascular Surgery 4 weeks (160 hours) June-July 2025
· During routine vital observations on a patient who had a lower limb amputation, I found out that the patient had a sudden drop in GCS. I promptly escalated this to the RN, and together we initiated a MET call. I also made sure to hand over to the shift coordinator and complete necessary documentation as per hospital policies while being supervised by my preceptor. This resulted in the timely stabilization of the patient, and appropriate interventions were put in place.
