Mental Health rotation:

• Managed a patient load of 26 beds across acute and sub-acute areas
• Conducted clinical reviews of paper drug charts to ensure adherence to medication safety protocols and NIMRC (National Inpatient Medication Chart) standards.
• Facilitated weekly patient education groups focusing on medication adherence strategies and the pharmacological management of various mental health conditions.
• Participated in weekly multidisciplinary clinical review meetings to optimise inpatient pharmacotherapy.
• Supported transitions of care to community mental health services (e.g. CCU, PARC, ORYGEN) for patients on opioid replacement therapy and clozapine.

Medical and surgical ward cover:

• Demonstrated adaptability and strong problem-solving skills while covering diverse clinical areas including cardiology, respiratory, general medicine, general surgery, and geriatrics.
• Facilitated efficient transitions of care via the transit lounge, ensuring timely discharge and continuity of medication management.
• Conducted Webster‑Pack trials for geriatric patients who had difficulty managing multiple medications, assessing impact on adherence, administration errors, and treatment outcomes.

Obstetrics and gynaecology ward cover:

• Assessed the appropriateness of medications for antenatal, postnatal, and breastfeeding patients.
• Evaluated the need for extended pharmacological VTE prophylaxis in high-risk patient cohorts and provided education at discharge.

Clinical education and mentorship:

• Acted as an intern preceptor, coordinating tutorial materials on OTC medications and delivering practice AHPRA registration exams.
• Provided mentorship and professional support to intern pharmacists through monthly one-on-one check-ins.

Clinical duties:

• Conducted medication histories & reconciled inpatient medications to identify clinical interventions and optimize pharmacotherapy.
• Conducted home medicines reviews under the guidance of the GEM@Home and HITH pharmacists to assess compliance & patient progress post discharge from an acute inpatient setting.
• Clinically reviewed a daily independent patient load of 15 to 20 beds in both acute and subacute medical and surgical wards.

Operational duties:

• Compounded aseptic parenteral and non-aseptic medications in accordance with PBA complex compounding guidelines.
• Dispensed inpatient, outpatient and discharge medications utilizing Merlin in accordance with hospital formulary and PBS requirements.
• Assisted with Outpatient pharmacy in the assessment, supply and counselling on a variety of specialized medicines for conditions including tuberculosis, Hep C, HIV and perioperative instructions for colonoscopy bowel preparations.

Quality use of medicines:

• Conducted two medication audits to assess appropriate storage, documentation and quality use of S4 and S8 medicines and compliance with legislative requirements.
• Accurately conducted TDM of high risk medications by reviewing dosage, dose timings and trough levels interpretation (vancomycin, tacrolimus).

• Compounded S4 medications whilst supervised by an accredited pharmacist including minoxidil, sildenafil and PEA capsules, ketamine lozenges, antibiotic suspensions and emollient creams
• Organized and packed 10 to 15 weekly Webster packs for regular patients utilizing SUREMED
• Provided basic wound care management for grazes and falls to elderly patients, majority of who were on blood thinning medications
• Utilized LOTS dispensing system to safely dispense S4 and S8 medications
• Regularly assessed appropriateness of medication regimens and recommended de-prescribing of high risk/unnecessary medicines.

• Clinical duties:
• Clinically diagnosed primary care conditions and provided S2/3 requests under the supervision of my preceptor
• Verified prescription information against safety guidelines and appropriateness in consideration of individualized patient conditions
• Performed blood pressure monitoring on patients and provided diabetes education, specifically blood glucose testing
• Operational duties:
• Trained junior pharmacy students on safe dispensing, S2 and 3 medications and administrative responsibilities
• Balanced cash registers and accurately calculated bank deposits
• Utilized Microsoft Excel to document daily budget reports.
