• Lead the clinical pharmacy service at Portland District Health across acute, sub-acute and outpatient services such as oncology, drug and alcohol and dialysis clinics.
• Review and update all policies related to the medication laws and regulations as well as the safe and effective use of medicine.
• Chair the Drugs and Therapeutics committee and provide clinical governance across all medication related inpatient and outpatient hospital services and activities.
• Provide education sessions for junior medical, nursing and allied health staff focusing on evidence based medicine and medication safety.
• Project lead for the implementation of Guidance- Antimicrobial Stewardship programme at PDH.

• Providing Pharmacist clinical insight and experience into the design and build of a new electronic patient management system.

Dual Role, spilt between

Oncology management

• Ordering and checking of chemotherapy products.
• Medication and techinical advice to medical and nursing colleagues including use of the Charm chemotherapy IT system.
• Advice to other pharmacy and medical colleagues looking after oncology patients on the wards.

Senior department/ward duties.

• Training of interns and new staff members.
• Teaching and support of the new medical HMOs and interns.
• Medication policy development.

• Departmental strategic planning for optimal delivery of clinical services.
• Recruitment, supervision and mentoring of junior pharmacists, interns and technicians.
• Working as part of the MDT to enable the provision of oncology and hematology services in Alice Springs
• Review of patients on initiation of chemotherapy to ascertain the suitability for therapy and any necessary dose adjustments.
• Provision of clinical and logistical oversight to the ordering of interstate chemotherapy.
• Preparation and checking of all chemotherapy and supportive medicines and counseling patients on the use of these
• Provision of education and advice to other areas of the hospital re oncology patients and cytotoxic agents.
• Financial management and budgetary reporting for the oncology department. Ensuring medication all PBS reimbursements are claimed and losses through excess or wasted stock are minimized.

• Provision of clinical pharmacy services in a cover role, stepping in where colleagues were off or unwell. This required a broad knowledge of all clinical areas as well as the ability to learn fast and understand the nuances of many different and distinct clinical areas including General Medicine, Emergency, Gem/Rehab, Cardiology, Orthopedics, plastics, gastroenterology Neurology and neurosurgery.

Rotations

Infectious Disease (HIV specialty unit)

• Education around treatment and prevention of sexually transmitted infections.
• Initiation of Anti Retroviral Treatment in a newly diagnosed HIV patient with comprehensive medication information and counseling.
• Treatment of the opportunistic infections that arise in those with low a CD4 count.
• Ensuring any medications prescribed for comorbidities are safe and preventing interactions or compromise of HIV treatment.
• Clinical guidance re switching antiretroviral therapy in those with a sub therapeutic response or intolerable side effects.
• Developing strategies to improve adherence in those with complex medication regimes and/or low health literacy.

Surgical ( Plastics and Ent/ Orthopedics/ Gen Surg)

• Peri operative comorbidity management.
• Selection of appropriate antibiotic prophylaxis and assistance with choice and duration of post-operative antibiotics.
• Provision of advice and alternative options for medication administration for patients via NG or PEG tube.
• Education and advice for medical staff and patients re anticoagulants as indicated post major joint replacement surgery.
• Counseling around Pain management strategies with documented weaning plans for any post operative opioids.·

Pharmacist led practice

• Nicotine Replacement Therapy provision and education for inpatients.
• Therapeutic drug monitoring and dose calculation of vancomycin and aminoglycosides
• Management and dosing of warfarin for inpatients.

A large, busy community pharmacy with a 300 km area of rural patient catchment.

• Full time role, including shifts as the sole charge pharmacist.
• Supervision and mentorship of pharmacy students, interns and technicians.
• Medication review and counseling for patients discharged from hospital and those in the community
• Provision of primary care services for mild to moderate accidents and ailments with referral to GP and or hospital as appropriate.
• Organization and oversight of the opioid replacement therapy program.

A 200 bed rural hospital serving the wider west coast of New Zealand’s South Island. Clinical rotations including general medicine, general surgery, paediatrics, geriatrics, psychiatry and maternal health.

• Clinical assessment and pharmacy review of inpatients especially at transitions of care. Medication rationalization and de prescribing prior to discharge.
• Provision of medicine education sessions, both and discharge and for outpatients, including giving the pharmacy section of pulmonary and cardiac rehab.
• Ensuring continuity of care through regular contact with the local GPs, community pharmacies, aged care and rehabilitation facilities.
• Operational duties including pharmaceutical compounding, stock management, outpatient dispensing and responding to medicines information enquiries.
