  • Working as part of a multidisplinary team focused on improving the function and quality of life of patients
  • GEM care patients have unique care requirements that can be complicated by their age, complex psychosocial situations and increased risk of falls
  • Associated with their reduced mobility, cognitive impairment and incontinence commonly seen in this patient cohort I worked to ensure that I minimised risk by implementing regular toileting, reducing clutter and ensuring all walking aids including glasses were available
  • Working alongside my peers to recognise early signs of deterioration associated with malnutrition and dehydration and putting in place food and fluid balance charts to monitor as well as actively encouraging patients to maintain good oral intake
  • Attending a Family Violence workshop aimed at recognising various forms of elder abuse and carer stress, I strived to create an environment where patients and families felt safe to speak out
  • This also includes esculating concerns through to the appropriate channels and advocating for my patients when appropriate.

Registered Nurse

Neurosurgery
02.2019 - 08.2019
  • The high acuity of the ward meant that I participated in multiple MET calls, each time improving on my ability to quickly assess patient presentations, relaying vital information back to the wider team and contributing to best patient outcomes
  • During my time at 4 South I was able to expand upon my previously learned knowledge of the brain and spine pathophysiology learning to link presenting symptoms with likely neurological deficits and using the appropriate pathways to escalate patient care
  • Many patients admitted to the ward are as a result of an accident
  • Due to the sudden nature of these accident patients and families often feel overwhelmed at their situation
  • In a supportive manner I would answer questions, explain procedures and provide reassurance, linking them in with additional services if required.
