Who is Taikura Trust in Auckland New Zealand

Taikura Trust, a charitable trust contracted by the Ministry of Health to provide eligibility screening, assessment, service co-ordination and budget management for disabled people under 65 years old living in the greater Auckland region.


My Role As Needs Assessor/ Service Coordinator at Taikura Trust

  • Completing comprehensive needs assessments for people with disabilities, from people under 65.
  • Meeting with families to gather information through needs assessment, and through needs assessment been able to identify Key areas of needs of the person.
  • Once then needs assessment has been completed, look at options of which external services that would best meet the persons needs.
  • Develop Service coordination plans and allocate funding accordingly.
  • Refer for specialist assessments, e.g. occupational therapy and physiotherapy to ensure that the persons abilities are maximised.
  • Providing Support Coordination within a Holistic Approach which views individuals as interconnected beings, recognizing that their physical, emotional, social, and spiritual well-being are all linked.
  • Completing proposals to ministry for group home funding when deemed appropriate. When a person can no longer live independently or with family as their needs have increased. Look at group home funding as a last resort when allocating support services.
  • Maintain high quality documentation and record relevant information in line with practice and processes.

Who is Framework Trust- Life Skills and Day Service (Forensic Setting)

Framework Trust provides support/Rehabilitation services to people who have committed offences and faced charges of those offences. Consumers who are not able to fulfil their full sentencing by court due to their Intellectual disability. (Unfit to Plea and therefore placed under) The Intellectual Disability, Compulsory Care and Rehabilitation Act, ID (IDCC&R) came into effect in 2003, and the National Intellectual Disability Care Agency (NIDCA) was established to implement and monitor the Act.


My role as Senior Intensive Support Coordinator was to:

  • Attending multidisciplinary team meeting to discuss service users level of skills for independent living and enter back into community from prison.
  • Recovery Support: To help consumers develop coping mechanisms, manage symptom and to engage in activities that promote well-being.
  • Goal Achievement: Work with consumers to identify their goals and develop plans to achieve them, whether related to living skills, employment, or community integration.
  • Collaboration: Working in partnership with professionals, such as psychiatrists, psychologists, and social workers, to develop and implement treatment plans.
  • Effectively leading staff towards positive results
  • Ensuring safe work practices and compliance with policy and procedures within bail conditions.
