- Conduct detailed physical and cognitive assessments upon admission, and regularly throughout the stay.
- Monitor vital signs, mobility, skin integrity, nutrition, fluid status, and medication response.
- Identify signs of clinical deterioration and escalate appropriately using ISBAR and MET protocols.
- Manage multiple chronic illnesses (e.g., diabetes, COPD, heart failure, and arthritis).
- Collaborate with geriatricians and allied health professionals to optimize ongoing treatment plans.
- Safely administer and monitor complex oral and intravenous medication regimens, including PRNs and controlled drugs.
- Monitor for drug interactions, especially in polypharmacy contexts common in elderly patients, educating patients and caregivers on new medications and potential side effects.
- Recognize and respond to delirium, providing person-centered care for residents with cognitive impairment.
- Provide compassionate care for patients with terminal illnesses or frailty.
- Administer end-of-life medications as prescribed and provide emotional support to families.
- Implement and adhere to strict infection control measures, especially due to frailty and immune compromise.
- Document assessments, interventions, incident reports, and handovers comprehensively, and maintain accurate and up-to-date records.
