Delivering high standards of holistic nursing care within the community to meet the needs of the client, carer and family.
Assessing, planning, implementing and evaluating nursing care. Including; wound management, stoma care, peripheral access devises, continence management, medication management, health promotion and education.
Liaising with other service providers to ensure seamless, cohesive care within the community setting.
Supporting and orientating new staff members.
Occasionally covering Level 2 RN positions as a Case Coordinator, involving case management of clients and staff.
Health promotion, screening and education.

Coordinating smoking cessation.
Planning and delivering immunisation programmes.
