• Assessment of patients with fluid overload due to Chronic Renal Failure, Assessment of and Cannulation of patient’s Arteriovenous fistulas, fluid assessment, management of acute hypotensive episodes, using Fresenius 5008 dialysis machines, problem solving, taking blood samples, Chronic Renal Failure management and education for patients
• Primary Nurse role which involves discussing monthly blood results with patients and educating/providing support where needed and documenting monthly nursing notes to ensure up to date information is communicated to the Renal Consultants
• ACCES LINK NURSE ROLE Involves programming regular access flows of the vascular fistula, the patients “life line” and following up with the results
• I will then assess if further action needs to be taken to ensure that the vascular fistulas with any concerns are communicated to the RAH vascular access team to ensure any interventions are performed in a timely manner to ensure patency is maintained
• This role also involves educating the patients about their own assessments of their fistulas, ensuring appointments and interventions are communicated to the patient and the Renal consultant
• I also assist in educating junior staff in vascular access assessment and cannulation skills
• Working in dialysis also includes supporting patients and their families palliative care transition, liaising with Renal Consultants and escalate care when clinically indicated
• Preceptor to student nurses and Graduate nurses who are undergoing Dialysis training, facilitating patients who wish to progress to home dialysis and involvement in their training.

• Admitting patients; performing ADLS; performing vital signs; wound care; medication management; care of palliative patients; liaising with patient’s family and friends; care of pediatrics patients; care of cardiac patients; performing ECG; performing venepuncture; discharge planning; monitoring of patients with diabetes; management of the deteriorating patient; documentation in patient records; assist with planning patient care plan according to nursing diagnosis and medical diagnosis; allied health referrals; mentoring student nurses; working in the Emergency department admitting patients, taking ECGs, performing venepuncture, handing over to the ward nurses using the ISBAR handover; Using a Team STEPPS approach to communicating with members of the health care team.

• Care of the elderly; assist in planning resident care plans; assist with regular care plan reviews; wound care; venepuncture; medication management; ordering of medical supplies; support of care staff; assist with education of care staff; ensure adequate staffing; liaise with and report to care manager via phone; assist with Doctors rounds; assist in auditing; documentation in residents case notes; admission of new residents; on call duties when needed.
