- Provided domestic support, meal preparation assistance, transportation and comprehensive 24/7 care to NDIS participants recognised to have limb amputations, schizophrenia, illicit drug addictions, borderline personality disorder, traumatic brain injuries, post-traumatic stress disorder, autism spectrum disorder, major depressive disorder, and varying other complex mental and physical illnesses.
- Implemented individualised care plans, facilitated daily activities, coordinated doctors' appointments and collaborated with health professionals to develop treatment plans for participants (i.e., risk management plans/behavioural support plans).
- Documented and collected required data for participant evaluation and NDIS compliance through records of medications administered, NDIS goals achieved, mileage logs, participant behaviours and presentations and all services provided. Following shifts a thorough handover was provided to incoming staff and supervisors to minimise care interruptions for participant.
- Maintained a physically active lifestyle alongside NDIS participants, inclusive of a regular exercise routine of weekly five kilometer ParkRuns, gym sessions and ocean swims.
- Participated in regular inter-disciplinary training sessions with supervisors, doctors and allied health specialists.
- Cleaned domestic homes, community housing complexes and NDIS Supported Independent Living properties (SIL) to ensure a safe and hygienic environment for all NDIS participants.
