• High-risk, in-crisis clients, Supported Independent Living (SIL), Psychosocial Recovery Support
• Provide support to clients who are experiencing severe mental health challenges and complex behavior, such as absconding, self-harm, suicidal ideation, and suicide attempts.
• Collaboration with the Mental Health Emergency Response Team (MHERT) & Local Hospital Emergency Department (ED)
• Complete incident reports and client case notes, attend and participate in staff handover at the end of each shift.

• Researched and Coordinated a Primary Care at Home Team of Medical Practitioners, including Satin Pathology, GP Down South, Palmerston AOD Services, Silver Chain, Peel Mental Health, and the Local GP, who worked alongside the client and his family at home. The client passed away in October 2023.
• Assisted with day-to-day operations, working efficiently and productively with all team members.
• Consent signed and authorisation to act on behalf of the client, as NOK.

• Completed Intakes & Exits, Informed Consent (to share), Rights and Responsibilities, Kessler-K5 & K10, risk assessments, and safety planning.
• FDV Key Worker- Provided person-centered, comprehensive 'wraparound' services, to address the needs of women experiencing severe mental health challenges, due to family domestic, and sexual violence, through trauma-informed care and recovery-oriented practices.
• As a Support Worker, I collaborated with community resources to provide psychosocial support and services, encouraging empowerment, working alongside consumers, through their recovery journey.
• Actively involved in enhancing skills through attendance at workshops, seminars, webinars, and online Nationally Recognised Training.
• Provided case management services, including referrals, advocacy, and follow-up communication.
• Recognise, respond, and de-escalate crisis situations, with consumers (CaLD) inclusive, experiencing severe mental health and complex behavioral challenges.

Provided almost 4 years to voluntary care, ensuring comfort and emotional well-being during this challenging period.

• Delivered end-of-life palliative care, decisions, and sharing care and responsibilities for her children, as Next of Kin.
