Key responsibilities

• Identify education needs of patient living with diabetes and provide education & ongoing support to for better diabetes management
• Providing inpatient and outpatient education, support and care to people living with diabetes and their families and/or carers.
• Provide holistic patient centred care to people living with diabetes, including T1DM, T2DM, GDM and other types of diabetes.
• Provide education and support to women with diabetes in pregnancy through individual or group education (New GDM or insulin start groups)
• Provide phone support to people with diabetes, including insulin titration, self-management education and support through programs such as SDEC, HITH & Better at Home DNE
• Provide education for patient to manage diabetes with Advance technologies such as continuous glucose monitoring
• Work as part of a multidisciplinary team that liaises with acute and community service to provide education and support for people living with Diabetes

Achievements

• Applied for ADEA Credentialing.
• Developed comprehensive diabetes knowledge through mentoring process
• Attended education sessions and ADEA state conferences to completed CPD points for ongoing professional development.
