Responsibilities:

Ÿ Deliver a comprehensive curriculum and provide student training in the follow:

1. Certificate IV in Leisure and Health (CHC43415)

2. Certificate IV in Disability (CHC43115)

3. Certificate III in Individual Support (CHC33015)

Ÿ Review delivery material and assessment tools

Ÿ Deliver educational programs and facilitates learning and enabling students

Ÿ Undertake assessment and evaluation of learning outcomes

Ÿ Comply with appropriate legislative requirements, organisational policies and promoting the principles of occupational health and safety, ethical practice and equal opportunity

Ÿ Maintain student records and documenting attendance, participation and progress

Responsibilities:

Ÿ Provide education, consultation and guide health staff within the Central Coast Local Health District

Ÿ Liaise and network with Ministry of Health, NSW Health, NDIS and other Local Health Districts

Ÿ Delivery of a multidisciplinary assessment and care planning for people with intellectual disability and complex health needs to build the skills, confidence and improved clinical and psychosocial outcomes

Ÿ Build capacity in local health services and assist with developing and delivering outreach services

Responsibilities:

Ÿ Develop implement and evaluate care management plans for patients with complex health needs

Ÿ Provide leadership in the ongoing development of clinical practice

Ÿ Initiate and utilise findings of research in the provision of clinical services

Ÿ Lead and contribute in the development and delivery of specialty related education programs

Ÿ Contribute to the strategic and operational planning for clinical services

Discharge planning through comprehensive assessment identifying any services, equipment or follow-up that may be needed to safely transition the patient home or elsewhere.

Assist with reducing hospital length of stay and unplanned readmission to hospital.

Support RACF staff by utilizing critical care and aged care emergency knowledge to support the residents to remain in their homes.

Utilizing clinical expertise with performing comprehensive and accurate nursing assessments of elderly patients with the ability to develop a plan of care in consultation with the patient, family, RACF and the multidisciplinary team that ensures continuity of care.

Responsibilities:

Ÿ Deliver and mentor specialist education and support to clinical employees

Educated patients and families on treatment, expected outcomes and discharge instructions, serving as advisory resource to patients and families to facilitate recovery.

Oversaw admissions and discharges and scheduling of nursing staff.
