Responsibility: My role focuses on providing supports that are relevant to the client`s needs, that fit within their individual NDIS goals.

Key accountabilities

· Implementing and participating in the creation of tailored care plans aimed at helping clients achieve their individual goals.

· Perform direct care duties not limited to preparing meals, personal care, gastrostomy feeding, medication administration.

· Manual handling

· Providing effective behavioural support and mentorship, guiding them to develop independent living skills.

· Completing any relevant client documentation.

· Provide support for Mobilising and moving clients using required equipment such as hoisting, wheelchair, Sara Stedy.

· Ensuring an opportunity for Client`s to achieve a quality lifestyle by enabling them to participate in community activities.
