+ Supporting clients to participate in age appropriate activities and tasks
Providing direct care assistance to each client in accordance with their service plan
Working in accordance with the medication administration policy, OHS procedures and client right's policies including privacy and confidentiality
Monitoring the safety and wellbeing of clients
Completing appropriate documentation
Working with clients, carers/families, relevant case managers to support the client's individual goals
Supporting clients in small groups as well as individually
Supporting clients in the community as well as in their own home, Including personal and practical care.
Supporting patients in feeding
Assisting patients with daily living
Assisting patient mobility
Patient toileting
Maintaining confidentiality, privacy and dignity of clients
Demonstrating empathy towards clients and relatives
Providing practical intervention procedures for dementia or behavioural problems
Assisting the nurse to settle patients for sleep and rest periods
Acting tactfully and diplomatically with sensitive situations
Demonstrating safety awareness and reporting safe issues and potential workplace hazards to the RN
Environmental maintenance including implementation of infection control standards
Ensuring all information and data regarding patients is recorded on appropriate charts and reported to RN as required
Completing client documentation
Ensuring organisational policies and procedures are adhered to at all times
