Summary

Registered nurse at medical ward or Rehabilitation and stroke ward in Internal Medicine Professional reflection of my post-graduate years to date has enabled me to formulate my career goals. My casual work experiences in non-medical nursing environments in private sector have fueled my interest in trying my responsibilities as a Registered nurse/ clinical nurse in new environment. Overview Now in my eleventh year as a Registered Nurse, I recognize the valuable grounding of clinical experiences acquired in the context of life-long learning. This grounding in rural to urban, and non-acute to acute environments, has been a layering of new skills on top of fundamental baseline life-long skills. I recognize my own growth as a professional, seeking and valuing constructive feedback from colleagues. I engage the consumer in their health care journey, asking them questions and providing opportunity for them to ask questions. I offer health education and advise to consumers, and embrace a team approach with other nurse colleagues, and the multidisciplinary team, striving for optimal patient outcomes.   • This 29 bed unit was formed within restructure - implemented 2 September 2013 - merging acute and subacute case mix into one multidisciplinary team and unit
  • I have proactively engaged in collaborative multidisciplinary care model, contributing to unique goals of acute stroke and rehabilitation patients
  • I continue to apply acute assessment skills in this unit, and follow through with early intervention strategies to promote optimum length of stay.

  • Acute Medical Unit, Ward 1G
  • As an RN in this 20 bed unit, I contributed to optimum outcomes for people with acute illness, many with single or multi-organ failure of mild to severe complexity
  • I contributed to care for people of all ages
  • The casemix in this unit required application of core nursing skills as well as advanced skills, with presentations including seizures, myocardial infarction, exacerbation / infective COPD, anaemia, refeeding syndrome, acute pulmonary oedema, unstable diabetes, polypharmacy overdose, acute liver failure, acute renal failure – to name a few
  • The unit also provided acute medical service for patients with mental health disorders requiring medical stabilisation before transfer to the MH specialty
  • This broad range of acute clinical casemix provided a platform for consolidation of pathophysiology supported by access to CKN and organizational policies and procedures to facilitate best practice
  • The broad range of casemix also provided application of numerous adjunct skills related to equipment, with the application of various infusion pumps (patient controlled analgesia devices, Gemstar subcutaneous infusion, Alaris infusion, syringe drivers, blood product delivery devices), naso-gatric feeding devices (via peg and naso-gastric/naso-jejunal tube), TPN regimes, humidified high flow nasal cannula oxygen delivery system, ‘Bear Hugger’ thermal regulating system
  • I added to my skill set with the wards’ embedded Close Observation Unit focusing on acute delirium on the older person
  • I established skills though internal training opportunities, and collaboration with the multidisciplinary team and the Medical Consultants.
