Continuing Care Team / Adult Recovery & Wellbeing Service

Leadership of the adult case management team, providing support and guidance to colleagues in fostering quality client care. Inclusive of line supervision, quality control activities and coordination of learning opportunities for multidisciplinary undergraduates and graduates.

Recent involvement in supporting the service's new Model of Care, as acting Manager.

• Head to Health. Providing holistic mental health assessment, support and counselling to community members, including referrals to appropriate services. Development of Head to Health outreach service to Ouyen/surrounds and supported Mallee Track Health & Community Services during establishment of their new mental health roles
• Suicide Prevention. Assessment and risk management for clients referred by their GP, who have expressed suicidal ideation or been discharged to GP care from hospital after a suicide attempt.
• Clinical Care Co-ordination.

• Assessment and support for clients with planned withdrawal from substances.
• Communication and co-ordination with external agencies to facilitate withdrawal, either in the home or hospital setting.
• Post withdrawal support and referral for clients as required.

• Co-ordinating the holistic care of adult clients with serious mental illness
• Ongoing assessment of mental state, risk assessment and management
• Monitoring of treatments, with focus on effectiveness and presence of adverse effects
• Continual communication within the team, other teams in the service and external agencies to ensure best quality care for supporting the client to achieve recovery
• Acting team leader role as required

• Provided case management to youth diagnosed with varied forms of diagnoses deemed early psychosis
• Intensive support and monitoring of clients symptoms and response to treatment
• Working on thorough developmental histories
• Liaising with the clients family, friends, educational institutions or employers to support maintaining best functioning
• Psychoeducation provided to the client and family
