Doorways

• Conducting interviews and assessment Australia wide for emergency relief (ER) with community members in accordance with Doorways guidelines
• Practice adherence with TSA’s Policies and Procedures, Doorways Model of Care and Practice Guides
• Actively support the local mission delivery model by working collaboratively with Corps
• Referring community members to appropriate internal and external services for additional support
• Completing daily administration duties using a range of software (SAMIS, Genesys and JIRA, GreenID, Centrelink, Uload, InfoExchange, Onesight Vision, Teams, SolvSafety)
• Requesting welfare checks when needed- liaising with Police
• Completing Child Protection report online or via phone when needed in any state
• Monthly supervision and check-ins
• Conducting frequent online mandatory trainings through E-Learn
• Liaising with Team Leader when service gaps are identified and providing recommendations for service delivery improvements
• Liaising with Senior Assessor when needed

Reconnecting Lives Program

• Providing outreach wrap-around services
• Working with consumers to help them achieve their goal
• Supporting consumers to live independently in the community
• Providing assistance and advice to maintain tenancy
• Linking consumers with their local community
• Providing employment assistance and advice
• Providing assistance with Disability and NDIS application
• Working collaboratively with internal and external services to support consumers
• Providing budgeting advice
• Providing assistance in crisis situation
• Making use of Housing-led and person-centred approach when supporting consumers

Future Homes Program

• Working within supported accommodation to support men experiencing homelessness
• Providing emotional, social and daily living support to consumers
• Following recovery support model to assist the consumers find long-term accommodation
• Completing daily administration duties using a range of software (SHIP, IXD and RISKMAN)
• Writing case notes in ISBAR format

• Assessing referrals from other services
• Liaising with services (non-profit organisations, prison, hospitals)
• Face to face and phone interview with potential consumers
• Completing admission process and paperwork with new consumers
• Creating profiles and updating information in client database (CHINTARO)
