My professional background is social work. I would like to apply for the position of disability support worker as a new graduated social worker. I have placement and volunteer experience in community neighbourhood centres, engaged in different programs. Brief experience in clinical settings and aged care facilities. I developed and strengthened various social work skills including in communication, demonstrating empathy, and supporting community engagement. I believe these skills are core in working with people with disabilities. I am passionate about supporting people living with disabilities.
· Provide services to clients with NDIS.
· Provide support advices and clients evaluation for the teams.
· Engage with new clients and report to organisation and share to teams.
· Help clients manage and reach individual goals, supporting independent progression and social skills.
· Assist clients with daily living needs to maintain self-esteem and build capacity.
· Transport clients to appointments, treatment centres and rehabilitation venues according to determined schedule.
· Medical routine maintenance, including liaising with allied health workers.
· Maintain safety with tidy, clean, and hazard-free home environments.
· Assist in daily living activities to maintain the daily routine.
· Provide psychosocial evaluation and emotional support.
· Participate in interdisciplinary care conferences to discuss patient care plans.
· Timely report incidents or other issues.
